Provision of Financial Assistance to Indigents
ABOUT THE SERVICE:
The Office of the Mayor provides financial assistance to indigents like
medical, burial and educational assistance.
REQUIREMENTS: ( Medical Assistance )
- Certication of Indigency from the Punong Barangay
- Medical Certificate
- Request letter addressed to Mayor
- Hospital Bill/ Certificate of Confinement/ Medical Prescription/ LaboratoryRequest
- Photocopy of Community Tax Certificate ( Cedula )
- Photocopy of any Valid Identification Card
- Proof of relationship of claimant to the payee
- Certification from Municipal Social Welfare Development Office

- Certication of Indigency from the Punong Barangay
- Death Certificate
- Request letter addressed to Mayor
- Photocopy of Community Tax Certificate ( Cedula )
- Photocopy of any Valid Identification Card
- Proof of relationship of claimant to the payee
- Certification from Municipal Social Welfare Development Office
REQUIREMENTS: ( Educational Assistance )
- Certication of Indigency from the Punong Barangay
- Certificate of Registration / Statement of Account
- Request letter addressed to Mayor
- Photocopy of Community Tax Certificate ( Cedula )
- Photocopy of any Valid Identification Card
- Proof of relationship of claimant to the payee
- Certification from Municipal Social Welfare Development Office
Securing Mayor’s Clearance
ABOUT THE SERVICE
Some schools and employment agencies require their students / recruits to secure a
Mayor's Clearance before they are allowed to enroll or apply.
These document can be secured from the Office the Municipal Mayor.
REQUIREMENTS
- Xerox Copy of Police Clearance
- Official Receipt from the Municipal Treasurer's Office
FEES:
Mayor’s Clearance - Php 100
Preparation of Disbursement Vouchers
ABOUT THE SERVICE:
Disbursement Voucher is a form used to have a check made to pay an
individual or an organization for merchandise sold or services rendered.
REQUIREMENTS:
- Official Receipt
